What Is NDIS Support Coordination?

Support Coordination is a capacity-building support funded through some NDIS plans. It helps participants understand their funding, choose suitable providers, coordinate services, and build the skills needed to manage supports more independently.

Your Support Coordinator works alongside you, your family, carers, therapists and service providers to help make sure your supports are organised, goal-focused and working well together.

Coordinate allied health, community, personal care and housing supports
Prepare for plan reviews and gather supporting information
Resolve service issues or gaps in support
Build confidence in making decisions about your supports
Explore SIL, SDA, MTA or other accommodation options where suitable

Support Coordination Built Around Your Goals

Every participant’s needs are different. Some people need help finding the right providers. Others need support coordinating complex services, preparing for a review, or planning a move into supported accommodation.

We take the time to understand your goals, preferences and current situation, then help create a practical pathway forward. You remain in control of your choices, while we provide guidance, coordination and support.
